Insurance and Payment Information

Payment for services is due at the time of your office visit. This includes insurance co-payments, deductibles, and any non-covered services. We accept cash, local checks and most major credit cards.

Charges are determined at the time of service based on several factors required by Medicare and other insurance companies. Certain tests and/or procedures may not be covered. To obtain your coverage levels, please call your insurance company before receiving care.

We participate with Medicare and most other insurance companies. In order for us to file your Medicare/Insurance claim for you, it is essential to bring your insurance card/s and driver’s license with you at the time of your visit.

Office Hours

Office visits are by appointment only. We know your time is important and we strive to stay on schedule. However, medical emergencies may create delays at times, so we ask for your understanding when they occur.

Emergencies

In an emergency, if the situation is critical call 911. If the situation is urgent but less critical, call our regular office number and the answering service will forward all messages to your doctor or the physician on call.
